What is the ACCESS Model?

ACCESS — Advancing Chronic Care with Effective, Scalable Solutions is a 10-year voluntary model launched by the CMS Innovation Center.

Today, more than two-thirds of Medicare beneficiaries live with chronic conditions like high blood pressure, diabetes, chronic pain, or depression. Traditional fee-for-service Medicare has not kept pace with how modern, technology-supported care is actually delivered. ACCESS changes this — by creating a payment pathway that rewards measurable improvement in patients' health, delivered through digital tools, remote monitoring, and flexible care models.

The Most Common Chronic Conditions

ACCESS focuses on conditions that together affect the majority of Medicare patients — organized into four clinical tracks that guide how care is structured and how outcomes are measured.

Paid for Outcomes, Not Activities

Participating care organizations receive recurring Medicare payments — called Outcome-Aligned Payments (OAPs) — for managing patients' chronic conditions. Payment is tied to whether patients actually improve: lower blood pressure, better HbA1c, reduced pain scores, improved mood.
